When you arrive to the office, please sign in and hand your paperwork to the receptionist.


What to Expect on Your First Visit


To make your first visit to the Institute of Sports Medicine and Orthopaedics run smoothly, you may download our office forms, fill them out prior to your visit and bring them with you to your first appointment.

If you were referred to our clinic by another physician, please bring in a request for consultation.

To ensure we have your information correct in our system, we ask that you are 15-20 minutes early, even if you sent in your paperwork prior to your visit.

At the Institute of Sports Medicine and Orthopaedics we strive to be as punctual as possible. If we are delayed for any reason, please know it is because each patient is treated with respect and care, and sometimes needs additional time.

If your visit is due to a hip, knee, ankle or foot problem, please bring a pair of shorts to wear during your exam. If not, you will be asked to change into a gown. If your visit is due to a back or shoulder problem, be prepared to remove your shirt (men), or change into a gown (women). If you are to be seen for an elbow or hand problem, please make sure they are easily accessible.

Once you have been escorted into an exam room, please make yourself comfortable. During your visit your doctor will take a complete history and perform a focused physical exam (with or without x-rays). He will then discuss the best course of action for your particular case.

Following your visit, the physician or assistant will walk you to the front desk where further instructions will be given, depending on the physicians recommendations.
